Observation, Documentation & Reporting builds the skill that ties all of caregiving together: noticing changes in a client, recording care accurately, and getting the right information to the right person at the right time. Caregivers are the eyes and ears of the care team, often the first to see that something has changed. This course covers what to observe (objective vs. subjective), recognizing and reporting changes in condition, how to document well, the legal nature of records, confidentiality, and communicating clearly with the nurse, supervisor, and team. It connects accurate records to client safety and to the Home Care Bill of Rights. The throughline: if it isn't observed and reported, the team can't act; if it isn't documented, it didn't happen.
