Writing outcomes that truly reflect the people you support is one of the most important — and most misunderstood — responsibilities of a Designated Coordinator. This workshop cuts through the compliance-versus-person-centered confusion and shows you how to do both at once.
You will explore what “person-centered” really means when it shows up in written outcomes, learn the practical tools for getting to know someone before you ever open a planning form, and see side-by-side exactly how an outcome can satisfy every 245D requirement while still sounding like it was written for a real human being with real goals.
By the end of this session you will have a five-step writing framework you can use immediately, a final test to run on every outcome before you submit it, and the confidence to recognize — and rewrite — outcomes that could belong to anyone.
This course covers:
- Person-centered practices for getting to know someone supported
- What “person-centered” actually means in the context of written outcomes
- The required elements under Minnesota Statute 245D.071, Subd. 4
- A practical five-step outcome writing framework with real examples
Audience: Designated Coordinators, Designated Managers, and anyone responsible for writing or reviewing outcomes under a 245D intensive support services license.
