Documentation is one of the most critical — and most audited — responsibilities a Direct Support Professional carries. This course covers the documentation standards required under Minnesota Statute 245D, including what to document, how to document objectively and accurately, how to correct errors properly, and how to handle late entries. You’ll learn the difference between compliant and non-compliant documentation, when a formal incident report is required in addition to standard shift notes, and what “if it wasn’t documented, it wasn’t done” means for your agency’s billing and compliance standing.
The second half of the course covers Electronic Visit Verification (EVV) — the federally mandated system Minnesota requires for all 245D personal care and home health visits. You’ll learn how EVV works, what data it captures, why it matters for Medicaid reimbursement, and what Minnesota’s compliance thresholds mean for you and your agency.
By the end of this course, you will be able to write objective, legally compliant service notes; correct documentation errors properly; identify events that require incident reports; and use EVV correctly on every eligible visit.
